I read this in the early/mid 2000's. They believe they are sisters, and little sister is sick all the time.
Older sister comes home from school to find out little sis is a princess and she was her bodyguard. She has powers and successfully holds the bad guys off until cavalry arrives. Little sister goes off to rule planet while she goes to assassin/body guard/thief academy where her family always goes.
There's also a boy named Tom (I think?) who was responsible for capturing the princess. He pretended to attend school with older sister. He refused to help capture her, and was punished by his employer by being turned into an owl.
